Telmisartan + Amlodipine + Chlorthalidone

Tablet

This triple combination provides comprehensive antihypertensive therapy. Telmisartan, an angiotensin II receptor blocker (ARB), lowers blood pressure by blocking vasoconstriction. Amlodipine, a calcium channel blocker, relaxes vascular smooth muscle. Chlorthalidone, a thiazide-like diuretic, reduces blood volume and enhances blood pressure control. Together, they provide synergistic control of hypertension and cardiovascular protection.

Strength
  • Telmisartan 40 mg
  • Amlodipine 5 mg
  • + Chlorthalidone 12.5 mg
Packing Size
  • 10 Tablets

Mechanism

Telmisartan: Blocks angiotensin II receptors, preventing vasoconstriction and aldosterone release.
Amlodipine: Inhibits calcium influx into vascular smooth muscle, causing vasodilation.
Chlorthalidone: Increases renal excretion of sodium and water, reducing plasma volume and blood pressure. The combination ensures effective blood pressure control and cardiovascular protection.

Pharmacokinetics

  • Absorption: Telmisartan ~50% bioavailability; Amlodipine ~60–65%; Chlorthalidone well absorbed orally.
  • Distribution: All highly protein bound.
  • Metabolism: Telmisartan minimal hepatic metabolism; Amlodipine via CYP3A4; Chlorthalidone minimally metabolized.
  • Elimination: Telmisartan mainly fecal; Amlodipine renal and fecal; Chlorthalidone renal.
  • Half-life: Telmisartan ~24 hrs; Amlodipine ~30–50 hrs; Chlorthalidone ~40–60 hrs.

Side Effects

  • Dizziness
  • Peripheral edema
  • Headache
  • Fatigue
  • Electrolyte imbalance (Chlorthalidone)
  • Rare: Hypotension

Contraindication

  • Hypersensitivity to Telmisartan, Amlodipine, or Chlorthalidone
  • Pregnancy (especially 2nd and 3rd trimester)
  • Severe hepatic impairment
  • Severe renal impairment
  • Cardiogenic shock (Amlodipine contraindicated)

Interaction

  • Other antihypertensives (additive effect)
  • CYP3A4 inhibitors (increase Amlodipine levels)
  • NSAIDs (may reduce antihypertensive effect)
  • Potassium-sparing diuretics (risk of hyperkalemia with Telmisartan)

Warning and Precautions

  • Use under medical supervision only
  • Monitor blood pressure and electrolytes regularly
  • Caution in renal impairment
  • Consult physician during pregnancy and lactation
